AVAILABLE IN MASSACHUSETTS AND NEW HAMPSHIRE

HMO (Health Maintenance Organizations) MA and NH
Members of the Harvard Pilgrim HMO (health maintenance organization) receive coverage for
routine and preventive care services with little or no paperwork such as bills or claim forms.
Care is typically covered in full after a small co-payment. The HMO plans are available to the
self-employed and small business owner. Harvard Pilgrim utilizes an extensive panel of doctors
available in a multitude of medical settings, such as individual practices, group practices and
large multi specialty groups.

HSA (Health Savings Accounts) MA and NH
The HPHC Insurance Company Best Buy HSA PPO is a plan that provides the opportunity
of reducing annual premiums. It also allows money to be put aside in a tax-deductible health
savings account to help offset medical costs below the plan deductible. The Best Buy HSA PPO
has in and out-of-network deductibles, while offering extensive freedom of choice of doctors and
hospitals without the necessity of a referral.

POS (Point of Service) NH only
Members of the Harvard Pilgrim POS (point of service) receive greater choice while still
receiving the advantages of a managed care plan. Members receive the same benefits as the
HMO but also have the opportunity to receive care outside of Harvard Pilgrim's physician and
hospital network. Members can see any provider without a referral. When self-referring,
members share the cost of health care through a plan deductible and coinsurance.

RETIREE HEALTH PLANS
One plan is available to group retirees and working employees that have
attained age 65.

FIRST SENIORITY (MA and NH)
First Seniority is a Medicare Advantage HMO health plan for people who
are entitled to Medicare or already enrolled in Medicare. Members of
First Seniority have access to over 16,000 physicians including 824 Primary Care.
The plan includes unlimited prescription drug coverage with only small-tiered co-payments.
It also allows access to any Medicare participating doctor in the United States.
Higher co-payments are charged for non-contracted providers.

EMERGENCY CARE
Members are always covered for care in a serious medical emergency. Members should receive
treatment at the nearest emergency facility, call 911 or another local emergency number.

WORLDWIDE COVERAGE
If traveling and in need of immediate care, members go to the nearest medical facility.
Harvard Pilgrim covers any unexpected or unforeseen care needed when traveling outside
of the service area.
